Foundation to financially help those in need with the costs of 
participating in youth soccer 
Phoenix, Ariz. - May 21, 2015 - AYSA is pleased to a nnounce that we have been selected as a recipient of the MaxInMotion® Foundation for the fourth year in a row!
Max Shacknai was the inspiration for MaxInMotion®. Max was a young, talented athlete who tragically lost his life in July 2011. The first word out of Max's mouth as a baby was "Ball!" His love of sports - soccer in particular - was known throughout the youth sports community. Inspired by his energy, spirit and commitment, MaxInMotion® is a not-for-profit foundation dedicated to providing young athletes an opportunity to compete at both the recreational and competitive levels in major youth sports.
The Foundation provides financial support directly to youth sports organizations throughout the state of Arizona to help underwrite inclusion in sports programs of financially challenged boys and girls who would not otherwise be able to participate. The Foundation embraces the philosophy that youth sports contribute dramatically to character development and personal fitness, and that team participation helps establish values of community, interdependency and collective responsibility. For those families who have participated in the last year, you are probably aware that in the first ye ar, we r eceived 164 applications. In year three, we received 799 applications! We fully expect to exceed that number this year.
New this Year: Club and Player applications must be submitted online.
To Begin the Process:
  1. Clubs must complete the online application - CLICK HERE
  2. Families must complete the online application - CLICK HERE
  3. Families must submit all required documents to their CLUB for submission to AYSA. The deadline for submission to AYSA is 12:00pm Noon on Monday, July 6, 2015. Please note: your club may have an earlier deadline for submitting your Player information, please contact them for verification. AYSA will only accept submissions from Clubs.
  4. Clubs will submit all required documents to AYSA on or before 12:00pm Noon on Monday, July 6, 2015.
Completed Applications
Once the online application is completed, submit required documents to your club. Each participating Club will then be responsible for submitting all documents to AYSA. The deadline for club submissions is 12:00pm Noon on Monday, July 6, 2015. Your club may have an earlier deadline for submitting your Player Application, please contact them for verification.
Very Importan t
Clubs have been instructed to thoroughly review and only submit complete player applications. This includes W-2 forms and tax returns/all related schedules. These documents (W-2 and tax returns must be submitted to your club, not directly to AYSA. If you are self-employed, tax returns with all related schedules will be required and likewise, submitted to your club.

Clubs will then submit all documents along with the Player Application and required documents to AYSA no later than Monday, July 6th at 12:00pm Noon. Please do not submit incomplete applications as they will be declined.

MaxInMotion® provides much needed financial assistance to youth sports programs for underprivileged children and for the development of programs to include special needs populations in youth sports activities.   Research shows that participation in youth sports enhances community values, interdepende ncy, and collective responsibility. Physical fitness prevents childhood obesity and has also been linked to reducing school dropout rates, teen pregnancy and high-risk behaviors, all of which pose tremendous consequences to our society.
MaxInMotion® has developed partnerships with other youth sports organizations, including Coyotes Charities, the Arizona Diamondbacks Foundation, Phoenix Suns Charities and the Arizona Bridge to Independent Living (ABIL).
